Intel (née Qlogic (née Silverstorm (née InfinIO))) 9024 SDR Infiniband switch. (There's a long story there and I'm not sure I have it totally correct, but... Infinicon/InfinIO renamed itself to Silverstorm at some point. Qlogic bought the whole company to get into the Infiniband business. Qlogic sold its Infiniband business to Intel in Early 2012.)

Firmware can (probably) be obtained from http://downloadmirror.intel.com/21914/eng/ (Thanks to Patrick Rutledge III for that info. I'd been looking for that for a while.)

Factory default username and password for this switch are admin and adminpass.

Management IP for this device is 172.16.0.249. Command line access is available over SSH. A web interface (over http and https) also exists.
