Qlogic (née Silverstorm (née InfinIO)) 9024 SDR Infiniband switch.

Firmware can (probably) be obtained from http://downloadmirror.intel.com/21914/eng/ (Thanks to Patrick Rutledge III for that info. I'd been looking for that for a while.)

Factory default username and password for this switch are admin and adminpass.
